Assessment, Partnership and Quality β Explained
Every care package begins with understanding the person, not just their condition. Here's how we build, deliver, and continually improve every care plan.
Getting to know you, before we write a single word of your care plan
Every care package begins with a comprehensive assessment covering:
From this assessment we develop a personalised care plan that is regularly reviewed and updated.
From first enquiry to ongoing care

Initial enquiry
You contact us by phone, email, or the enquiry form. We listen to your situation and answer your initial questions.

Comprehensive assessment
A member of our team visits (or speaks with) the individual to assess medical needs, physical health, mental wellbeing, communication, mobility, medication, nutrition, personal preferences, risks, cultural requirements, religious beliefs, and desired outcomes.
-
Personalised care plan
We build a care plan around what matters most to the individual, agreed with them, their family, and any relevant professionals.
-
Careful carer matching
We match individuals with carers suited to their personality, interests, and support needs, wherever possible maintaining consistency of staff.

Common questions about starting care
How quickly can care start?
Timescales depend on individual circumstances and the type of support required. Following your initial enquiry and assessment, we aim to begin care as promptly as possible, including urgent hospital discharge packages where needed. Contact us to discuss your timeframe.
Do you provide care outside of the West Midlands?
We proudly cover the whole of the West Midlands. If you're just outside this area, please get in touch β we may still be able to help or point you towards a trusted provider nearby.
Will I always see the same carer?
Wherever possible, we match individuals with a small, consistent team of familiar carers who understand their preferences, routines, and personality, rather than a constantly changing rota.
Is Frias Gate Care regulated?
Yes. Frias Gate Care is registered with the Care Quality Commission (CQC) β Registration No. 1-19493256716. You can view our current registration, ratings, and any inspection reports at any time via the CQC website.
